Everquest Utilities Page
It's a Windows Console based program (which means that there's no pretty GUI, you have to access it through a command window.)
In most operating systems, Start - Run - cmd (or command)
Change directory into your Everquest Logs folder (cd "\program files\everquest\logs") (quotes needed if there are any spaces in the path, I don't think it will work in 98/ME, you'll have to use the 8-character shortcut (progra~1) for long path names with spaces)
Place the Hero.exe file within the ZIP file in the Logs folder (you can use the Windows GUI for this and WinZIP)
Then in the command window, with the active directory being the EQ logs folder, you can run HERO.EXE by itself to get the usage:
USAGE: hero.exe <Your character's Name> <Your character's Server>
It will read in the log file based on your inputs, and output to the console all of the "Heroes". It will also create a file in your logs folder named "<Server Name>'s Heroes.txt" in case it's a large list, you can still access the output.
DEAD does the same thing HERO does, except for it counts the number of times that NPCs and PCs have died. The setup and usage are identical to HERO.
BARD does the same thing the previous two does, except for it counts the number of times that the individual lyric /says done to the event NPCs fail.
